Also complete the Human Questionnaire for yourself and a copy of the Animal Questionnaire for each animal you would like me to work with during your session. Because our animals frequently mirror our issues, please submit a questionnaire for yourself even if you only want me to work with your animals. You only need to submit one copy of the last page. Also submit a picture of your animals by email or by mail.

Please fax the forms to Margaret at 423-266-3384 or mail them to the address below for distant healing sessions. You may bring the form with you for an in person session.
